I do have a QT set up, but I don't let it run 24/7.  Thankfully, I haven't had any problems to where I had to transfer any of my existing fish over.  My QT is ONLY, at this point, for new members.  I will start the QT up, drip acclimate the fish, and in they go for 4-6 weeks.

my QT consists of a 20g tank, a heater, small power head, HOB filter w/ bio wheel, and pillow floss for the media.  that's it

I have a 40 breeder set up as a 24/7 QT. Using an emperor 400 HOB filter. Just finished 30 days of Cupramine on 2 fish and never had any problems with ammonia.

Same set up as heinbaughb with an addition of a powerhead that keeps the surface rolling for better air exchange. Did a group of med. size butterflies for over 60 days with no issues. Used coppersafe and treated for infections twice. Other then an occasional water change no issues. You can pre-seed sponges from your hob filter with bacteria to help getting the tank ready to deal with amonia from the fish.
