Help with clam

My clam has been in my tank for over 2 years now and has been looking good and growing. Today I came home to him laying on his side and when I stood him up he layed back down. Not sure what is going on

Is the mantle staying retracted? Can you see anything on it? Also, in the past, when it looked good, was the mantle fully extended, with no pinches.

It is out a little bit nothing like it used to be. Yesterday it was nice and extended with no pinches right now it looks like this this. It is like the two halves of the shell are sliding past each other and not staying even

Can you get him to close up all of the way and take a picture from the top, and then pick him up and take a picture of the bottom. But I cannot say that I've seen anything like this. My clams are so wide, they couldn't fall over.
